Information about St. Xenia Camp 2009 for Parents

Camp Dates

Start:  Sunday, August 16th at 3:00 P.M. (no earlier please)

Close: Saturday, August 22nd at 10:00 A.M. (no later please)

Parents and guardians are asked to pick up their children between 10 and 11 A.M. sharp!  Please plan to arrive on time.  

Importance of Camper Registration Payment

It is very important that you get your registration payment in as soon as possible. A non-refundable deposit of $50.00 can reserve a place, (for one camper), in the 2009 camp roster.  The $50 deposit will be applied to the remaining balance. The money from camper registrations is used to fund pre-camp necessities.  All of the planning, purchasing and coordinating have to be done months in advance.  Full payment will be required by June 30th.

·         $250  4th Camper  

Health Forms and Doctor Appointments

Health Forms must be submitted no later than July 31st. The processing of all the forms by the Camp Doctor is a time consuming task; therefore it is essential that all forms be submitted as early as possible, and no later than the deadline stated. A camper will not be able to attend without the submission of a Health Form. 

 

 

IMPORTANT!

 

A physical examination by a licensed physician is required every two (2) years. If an exam was done for last year’s health form, the physician’s section on this year’s form can be left blank. Parents, please fill out the remainder of the form. If a physical examination was not done last year, seeing a doctor for a new examination is necessary. If needed, make your doctor appointments soon! It often takes months to get in to see a doctor for a physical exam.

It often takes months to get in to see a doctor for a physical exam.  The Health Forms must be submitted before a camper can attend camp.  Please get your forms in soon.  The processing of all the forms by the Camp Doctor is a time consuming task and any help is appreciated to get these in as soon as possible!


*This year, we are no longer using the ACA medical forms, and we've created our own that can be "downloaded" and "printed" from the website. Although all the remaining registration requirements can be accomplished ONLINE, the Camp Medical Form and the Parental Consent Form need to be filled out and signed by hand, and then mailed to Catrin Thorp, the camp Registrar.  The medical form requires the first (3) pages to be filled out by all campers / parents. The fourth page is the doctor's section, which needs to be updated every (2) years. These forms are an absolute pre-requisite for camp registration and participation.
