*This year, we are no longer using the ACA medical forms, and we've created our own that can be "downloaded" and "printed" from the website. Although all the remaining registration requirements can be accomplished ONLINE, the Camp Heath Form and the Parental Consent Form need to be filled out and signed by hand, and then mailed to Catrin Thorp, the camp Registrar. The medical form requires the first (3) pages to be filled out by all campers / parents. The fourth page is the doctor's section, which needs to be updated every (2) years. These forms are an absolute pre-requisite for camp registration and participation.
Registration
Checklist:
1.
Fill out a Check for the Registration Fee of $50 deposit [deposit will be applied to the
balance
remaining]. Checks are to be
made payable to HOMB [notation: St. Xenia Camp].
2. Online Camper Registration (as noted on this link==>How to Register). Each camper must be Registered Online.
3. Health Forms (All campers must have this filled out. The Health Form must be submitted no later than July 31st. Campers will NOT be permitted at camp without a completed health form.). These forms can be printed off from this website - link==> Camper Forms.
4. Signed and completed Parental Consent Form. These forms can be printed off from this website - link==> Camper Forms.
Make sure you mail the following items to the Camp Registrar:
1. Completed "Health Form"
2. Completed and signed "Parental Consent Form"
3. Check for $50.00 as a deposit
